Well Water Test Form (PDF)

The above link will provide you with a Water Testing Company's contact information, as well as their form to fill out which must accompany the well water specimen at the time of pick up.  Please note you will need to deal directly with the Testing Company as well as pay them directly.

Our office can provide these aspects:

• The Public Works Department can provide a 'sanitary bottle' to use for your well water specimen, and,
• will provide you with a pick up time from our Public Works location rather than drive to Napa.

You must obtain the well water specimen the morning of the scheduled pickup and keep it refrigerated until it is brought to the office
